Job Title: LPN Rehab

Location: Chambersburg PA 17201
Duration: 3 Month Contract (Extendable)
Shift: Nights 312 hrs 36 hrs/week Every Other Weekend Required

Compensation:

• Travel: $51.61 / 36 hours

• Local: $44 / 36 hours

Job Summary:

The LPN Rehab provides patient-centered nursing care in a rehabilitation environment supporting patients recovering from injury illness or surgery. This role focuses on promoting mobility independence and functional improvement while collaborating with a multidisciplinary care team.

Responsibilities:

Provide skilled nursing care to patients in a rehab setting with a focus on recovery and mobility.
Administer medications injections and treatments per provider orders and facility protocols.
Monitor and record vital signs pain levels and patient progress; promptly report concerns to supervising RN or provider.
Assist patients with activities of daily living (ADLs) transfers ambulation and proper use of assistive devices.
Perform wound care dressing changes catheter care and other specialized treatments as required.
Collaborate with PT OT SLP and physicians to implement individualized care plans that support patient goals.
Educate patients and families on rehab routines medication use safety practices and discharge instructions.
Document care thoroughly in EMR systems and ensure compliance with state federal and facility guidelines.

Licenses & Certifications:

Active LPN License Pennsylvania
BLS (AHA)
Two professional references
COVID-19 vaccination card
