Position Summary

Reporting to the Manager of Community Nursing Services the Nurse Consultant provides nursing consultation to people across the age continuum referred to Nursing services when appropriate. The Nurse Consultant plays an active role in health education and in the assignment or delegation of tasks to unregulated care providers. The Nurse Consultant identifies ways to improve health care services for the people supported by the program. Nursing Consultative services are designed to support the care teams at each site or in a specific community.

Qualifications

Education:

  • Registered Nurse or Registered Psychiatric
  • Nurse Baccalaureate Degree required
  • Current C.P.R. skills at Basic Rescuer Level
  • Non-Violent Crisis Intervention is an asset

Professional:

  • Current registration in good standing with the CRNM or CRPNM required
  • Minimum of 5 years of clinical health experience including at least 3 years experience working with individuals with developmental and physical disabilities/dual diagnosis living within a community setting
  • 2 years in a formal supervisory or leadership role
  • Previous staff training experience and familiarity with adult-learning concepts and training techniques are an asset
  • Familiar with relevant legislation and regulations e.g. Regulated Health Professions Act (RHPA) Adults Living with a Disability Act (ALIDA) Personal Health Information Act (PHIA)
  • Proficient word processing and computer skills in Microsoft Office environment

Clinical:

  • Comprehensive knowledge of nursing theory and evidence-based practices relevant to the care of persons with a developmental disability
  • Knowledgeable of the history of services to people with disabilities and current trends philosophies and practices concerning their care education and lifestyles including cultural commensurability in indigenous health
  • Understands and supports the concept of interdisciplinary teamwork
  • Excellent written and verbal skill
  • Committed to ongoing personal professional growth and learning

Personal:

  • Functions effectively as a member of a team
  • Able to work independently and manage time effectively
  • Flexible and able to work a non-conventional schedule
  • Ability to travel for work purposes over single or consecutive days using multiple modes of transportation (vehicle plane water craft)
  • Valid Manitoba Drivers License in good standing
  • Must have access to a reliable vehicle


Duties & Responsibilities

Clinical Care Management:

Consistent with the Mission and Philosophy of the the Nurse Consultant is responsible for overseeing the provision of services within their area of clinical practice and ensuring adherence to and Program policies and the requirements of their professional regulatory body.

Duties include:

  • Becomes familiar with each persons health history and assesses current health status
  • Develops a health care plan for each person referred as required
  • Temporarily provides direct nursing intervention when necessary and possible until appropriate community referral/connection is made
  • Provides leadership and consultation during and/or following a medical emergency
  • Works closely with the leadership of program areas to assess training needs of direct care staff
  • Identifies trends in health needs of individuals/vulnerable populations and relays this to Manager Community Nursing
  • Promotes health and prevention of illness through sharing of information with the community and program leadership
  • Monitors whether health resources in the community are responsive and relevant to needs of individuals
  • Audits program to ensure health care management standards are maintained
  • Develops and implements an efficient training package for the individual(s) and their care providers with regard to general as well as specific health issues
  • Provides consultation and supports coordination of services to address health problems that have not been resolved by standard measures or that are complex in nature
  • Communicates effectively with the individuals substitute decision maker and other service providers as necessary and as requested by the program leadership
  • Provides recommendations for required medical follow-up to health-related incidents reported by the program staff
  • Develops nursing goals and objectives and focus statements for individuals within their program or service area part of their overall treatment plan
