About the Role

You will:

  • work within a flexible interdisciplinary structure that ensures the delivery of high quality therapy services to all clients of HIP Young Adults Complex Disability Service (YACDS)

  • provide evidenced based high quality physiotherapy services for clients of YACDS.

  • Provide clinical assessment and treatment for all assigned clients as appropriate according to HIP and St. Vincents policies and procedures and relevant service protocols

  • Collaboratively work with the multidisciplinary team members referring professionals community professionals and other key stakeholders within a flexible multidisciplinary structure

  • Ensure evidence-based best practice is delivered


Classifications for this position will range from classification range VB1 - VB4 ($45.66579 - $52.49211 per hour 25% casual loading)


Your Contribution

  • Bachelor of Physiotherapy AND minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or disability and/or community based service

  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Demonstrated sound knowledge and application of therapy practice and principles particularly in the areas of Disability Spasticity management and Neurological Rehabilitation

  • Experience or keen interest in postural management and wheelchair prescription

  • Clinical experience in the area of complex neuromuscular pathologies particularly cerebral palsy.

  • Knowledge and understanding of the NDIS and other relevant community resources and services

  • Current National criminal history check or willing to obtain/ Current NDIS Worker Screening Check or willing to obtain

The Young Adult Complex Disability Service (YACDS) aims to help patients their families and carers to transition into the adult health care system from childrens health services. The YACDS is available to adults aged under 40 with a history of developmental disability such as cerebral palsy or other medical conditions. Intervention provided includes assessment treatment and short term management which is time-limited and goalorientated.

Position Description

This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow through delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centred care planning. SVHM HIP consists of integrated hub sites across several locations including in Fitzroy Kew North Richmond and Fairfield.

The Young Adult Complex Disability Service (YACDS) aims to help patients their families and carers to transition into the adult health care system from childrens health services. The YACDS is available to adults aged under 40 with a history of developmental disability such as cerebral palsy or other medical conditions. Intervention provided includes assessment treatment and short term management which is time-limited and goalorientated.

This position may require working from any of the SVHM Health Independence Program (HIP) sites and may be required to work within any of the HIP programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH)

PURPOSE

To work within a flexible interdisciplinary structure that ensures the delivery of high quality therapy services to all clients of HIP Young Adults Complex Disability Service (YACDS) making sure that the clients needs are met and they receive the right treatment in the right place at the right time.

To provide evidenced based high quality physiotherapy services for clients of YACDS. Clinicians are required to provide assessment and management of clients in a number of settings across the continuum of care within HIP including clients residence community settings the HIP Centres or Specialist Clinics.

The primary purpose of this casual position is to provide leave cover for Grade 2 physiotherapy within the YACDS team across the calendar year.

5. POSITION DUTIES

  • Provide clinical assessment and treatment for all assigned clients as appropriate according to HIP and St. Vincents policies and procedures and relevant service protocols
  • Collaboratively work with the multidisciplinary team members referring professionals community professionals and other key stakeholders within a flexible multidisciplinary structure
  • Complete client goal setting in consultation with patient/family/carers as appropriate. Goals are to be functional and measurable
  • Evaluate treatment / management using the most appropriate methods making use of objective measures where possible
  • Ensure evidence-based best practice is delivered
  • Maintain current knowledge and skills in clinical best practice and disability and the National Disability Insurance Scheme (NDIS)
  • Supervision of undergraduate allied health students as appropriate
  • Provide guidance support and direction to Allied Health Assistants as appropriate
  • Contribute to service and stream development as appropriate to the cover period including ongoing data collection and outcome measures.
  • Participate in relevant in-service programs and meetings. Complete allocated tasks on time
  • Meet activity requirements as agreed upon with Team Leader
  • Maintain accurate statistics and submit in timely manner as per guidelines
  • Maintain the appropriate standard of medical record keeping and data collection as per St Vincents and HIP policies

6. INCUMBENT OBLIGATIONS

8. SELECTION CRITERIA

8.1 ESSENTIAL REGISTRATION LICENSE OR QUALIFICATION REQUIREMENTS

  • Bachelor of Physiotherapy
  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Eligibility for Australian Physiotherapy Association membership.
  • Current Victorian Drivers License

8.2 OTHER ESSENTIAL REQUIREMENTS

  • Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation and or disability and/or community based service
  • Demonstrated sound knowledge and application of therapy practice and principles particularly in the areas of Disability Spasticity management and Neurological Rehabilitation

  • Understand and support the principles of HIP continuum of care for clients particularly in relation to individualized time-limited goal-centred episodes of care that aim to improve health outcomes
