Job Summary
Synergy Care is looking for a Physical Therapist Assistant to fill a full-time position in Prophetstown IL. This is an onsite position at Winning Wheels a SNF.
Position Summary:
The Physical Therapist Assistant (PTA) works under the supervision and direction of a licensed Physical Therapist (PT) to provide rehabilitative services to patients with physical injuries illnesses or disabilities. The PTA helps implement treatment plans assist patients in improving mobility and function and supports their overall recovery and wellness.
Key Responsibilities:
Patient Care & Treatment
Assist the Physical Therapist in implementing individualized treatment plans.
Guide patients through therapeutic exercises mobility training and functional activities.
Monitor patients during sessions and modify activities based on their performance and feedback.
Documentation & Communication
Document patient progress treatment responses and any changes in condition.
Communicate regularly with the supervising Physical Therapist regarding patient status and adjustments needed for the treatment plan.
Maintain accurate and timely records in compliance with facility and regulatory standards.
Patient Education
Teach patients and their families exercises safety protocols and techniques to support recovery at home.
Provide instruction in the use of assistive devices such as walkers canes and braces.
Qualifications:
Associate degree from an accredited Physical Therapist Assistant program.
Current PTA license or certification in the state of employment.
Strong communication documentation and interpersonal skills.
Ability to assist and motivate patients with varying physical abilities.
