Job Title: Physiotherapist Home Reablement & Rehabilitation
Banding: Band 6
Location: South-West London (Kingston Wandsworth & surrounding areas)
Start Date: Early January
Duration: Approximately 3 months
Hourly Rate: 26-30 per hour
Working Hours: Full-time (weekday and weekend availability within a 7-day service)
We are currently seeking a dedicated and experienced Physiotherapist to join our clients team in South-West London. This is a fantastic opportunity to provide short-term home-based rehabilitation for patients recently discharged from hospital.
Key Responsibilities
You will be responsible for conducting physiotherapy assessments in patients own homes delivering short-term reablement interventions (typically up to 3 days per patient) and developing safe goal-focused rehabilitation plans. You will work closely with wider support teams to ensure effective handover to longer-term community services maintain accurate clinical records and contribute to maintaining high standards of patient care.
Qualifications and Experience
HCPC registration (Health and Care Professions Council)
Degree in Physiotherapy
Experience in community rehabilitation home-based assessment or reablement essential
Strong clinical reasoning and communication skills
Ability to work independently and as part of a multidisciplinary team
Previous NHS experience preferred
