Synergy Care is looking for an Occupational Therapist Assistant to fill a full-time position at Winning Wheels a SNF in Prophetstown IL. You must be a licensed therapist or a new graduate.
Key Responsibilities
Patient Care & Treatment
Carry out treatment plans established by the Occupational Therapist using therapeutic activities and exercises.
Assist patients with activities of daily living (ADLs) including dressing grooming feeding and mobility.
Utilize adaptive equipment and assistive technology to enhance patient independence.
Encourage patient motivation and provide support throughout the rehabilitative process.
Documentation
Maintain accurate timely and complete treatment records in accordance with facility policies and regulatory requirements.
Document patient progress goals and outcomes for review by the supervising Occupational Therapist.
Collaboration & Communication
Communicate regularly with the supervising Occupational Therapist regarding patient status and modifications to treatment plans.
Participate in interdisciplinary team meetings when appropriate.
Educate patients families and caregivers about therapy techniques and home exercise programs.

In order to be considered for this position you must be a graduate of an accredited school for Occupational Therapy Assistants or will graduate from such an institution within the next six (6) months. You must hold a valid National Board for Certification of Occupational Therapy (NBCOT) certification and you must currently hold or be eligible for a valid state license (where appropriate) as an Occupational Therapy Assistant.
