Job Summary
Occupational Therapist Assistant / Rehab Director (Full-Time)
Winning Wheels SNF Prophetstown IL
Synergy Care
Synergy Care is seeking a dedicated Occupational Therapist Assistant (COTA) / Rehab Director to join our team full-time at Winning Wheels a skilled nursing facility in Prophetstown Illinois. We welcome both licensed therapists and new graduates to apply.
Key Responsibilities
Patient Care & Treatment
- Implement treatment plans established by the supervising Occupational Therapist.
- Utilize therapeutic activities and exercises to improve patient function.
- Assist patients with activities of daily living (ADLs) including dressing grooming feeding and mobility.
- Apply adaptive equipment and assistive technology to promote patient independence.
- Encourage and motivate patients throughout the rehabilitation process.
Documentation
- Maintain accurate timely and compliant treatment documentation.
- Record patient progress goals and outcomes for review by the supervising Occupational Therapist.
Collaboration & Communication
- Communicate regularly with the supervising Occupational Therapist regarding patient status and treatment modifications.
- Participate in interdisciplinary team meetings as appropriate.
- Educate patients families and caregivers on therapy techniques and home exercise programs.
Qualifications
To be considered candidates must:
- Be a graduate of an accredited Occupational Therapy Assistant program (or graduating within six months).
- Hold a valid NBCOT certification.
- Hold or be eligible for state licensure as an Occupational Therapy Assistant.
