About the team

The Corewell Health (CH) West Wound Ostomy & Continence Team (WOC) is a program of registered nurses specially trained in caring for abdominal stomas dermal wounds pressure injuries incontinence and related skin conditions. The teams utilizes evidence-based practice standards to determine the most appropriate care for each patient. They are also an integral part of education for patients. They educate patients and families on proper care techniques empowering them to manage their conditions effectively at home. Through their compassionate and skilled approach the WOC team significantly contributes to patient recovery and overall well-being. This team focuses their efforts at the medical center and Blodgett campuses as well as staffing an outpatient ostomy clinic. Additionally the CHGR West WOC is involved on many unit and system initiatives to help reduce patient harm.

Essential Functions

  • Assesses and monitors the overall physical needs of the patients/family in regard to ostomy wound and continence. Evaluates each individual patients response to treatment.

  • Assists in developing/maintaining an individual plan of care with the patient consistent with changing needs and goals reflecting full scope of practice.

  • Collaborates in discharge planning for patient at time of patient contact including families caregivers as appropriate. May facilitate a smooth transition to the next point of service/home.

  • Assesses the educational needs of a select group of patients caregivers and staff. Develops and implements appropriate teaching strategies and makes appropriate referrals for patients providing counsel to enable them to adjust to living with a specific medical condition. Documents nursing care/teaching consistent with documentation guidelines. Ensures that required documentation for activities within area of responsibility is performed in an accurate and timely manner.

  • Collaborates with nurses providers and other departments to develop implement and evaluate programs and services that support patient care delivery in Wound Ostomy and Continence Nursing. This includes effective utilization of workforce equipment and supplies. Develops interdisciplinary relationships to support improvements in care delivery and patient staff and provider satisfaction.

  • Collaborates with nursing quality department to develop and maintain quality monitoring system to continuously monitor and measure the quality safety and outcomes of nursing services for assigned area. Develops and implements unit quality indicators to evaluate performance and the level of practice for Wound Ostomy and Continence nursing as well as develops/implements improvement plans to correct variances.

  • Participates in the development and review of policies procedures and guidelines related to wound ostomy and continence based on evidence-based practice research findings and Spectrum Health quality outcomes.

  • Encourages an environment supportive of nursing research promoting an evidenced-based clinically effective nurse-sensitive patient outcomes for Wound Ostomy and Continence Nursing.

Qualifications

  • Required Bachelors Degree Nursing

  • Experience teaching patients (pediatric and/or adult) families and other health professionals Required

  • Successful completion of competency based orientation program Required

  • 1 year of relevant experience relevant experience Clinical Nursing Preferred

  • LIC-Registered Nurse (RN) - STATEMI State of Michigan Upon Hire required

  • CRT-Wound Care Certified (WCC) - NAWCO National Alliance of Wound Care and Ostomy 2 Yrs after hire required Or

  • MAGCRT-WOUNDOSTOMYNURSE-WOCNCB #N/A 2 Yrs required

  • CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required Or

  • CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days preferred
