Job Summary
Job Title: Physical Therapist Home Care
Location: Wall Township NJ 07753
Schedule: 40 Hours/Week 8 Hours/Day 5 Days/Week
Shift: Day Shift (5x8)
Pay Rate: $60 - $63/hr
Position Summary:
Navitas Healthcare LLC is seeking Physical Therapist Home Carefor an exciting job in Wall Township NJ.
Responsibilities:
Conduct thorough patient evaluations using a holistic assessment approach
Develop and implement individualized treatment plans with measurable short- and long-term goals
Provide skilled physical therapy interventions based on patient needs
Prepare and submit progress notes reassessments and discharge summaries accurately and on time
Ensure documentation supports the medical necessity for continued skilled care
Follow frequency and duration outlined in the physicians plan of care
Serve as a liaison between physicians managed care providers and the healthcare team
Supervise and evaluate Physical Therapist Assistants (PTAs) and complete required co-signatures
Educate patients and caregivers on home exercise programs and safety procedures
Participate in discharge planning throughout the patients course of care
Adhere to organizational competencies and professional behavior standards
Meet physical job demands including lifting pushing and pulling up to 50 lbs. and standing for extended periods
Required Skills & Experience:
Minimum 6 months 1 year of Physical Therapy experience
Strong assessment and intervention skills
Proficient in electronic documentation and multitasking
Ability to complete documentation accurately and on time
Excellent communication and interpersonal skills
Home care experience strongly preferred
Education Requirements:
Bachelors Degree in Physical Therapy Required
Doctorate in Physical Therapy (DPT) Preferred
Required Certifications & Licensure:
Registered Physical Therapist Required
BLS Certification (AHA) Required
Certified Home Care Experience (Part A) with OASIS knowledge Required
