Job Summary
The Clinical Hospice Director is a great opportunity for an RN leader with experience and passion for hospice and end of life care. It could be your next step into a management role if you have hospice experience as an RN have management/supervisory experience even if it is not in hospice and want to lead. It is a good opportunity to join a reputable mission driven hospice agency for an experienced Clinical Hospice Director will:
Provide the supervision management coordination and provision of quality patient care
Collaborate with the physician referral sources Clinical Director and professional staff regarding patient eligibility for the hospice program
Coordinates hospice care services from admission through bereavement.
Coordinates with physicians case managers the patient families and interdisciplinary team to implement that patient plan of care.
Coordinates regular interdisciplinary team meeting regarding the patient care process to ensure quality of care and coordination of services.
Participates in agency quality assurance activities designed to improve quality and continuity of patient care
Oversees staffing training and evaluating staff.
Requirements include
3 years as a nurse and I year plus in patient care of hospice home health or oncology patients.
1 year of supervisory preferably in hospice or home health care.
Demonstrated ability to provide a supportive approach to staff and patients along with their families.
