Primary Care Physician Full-Time
Overview:
We are seeking a Primary Care Physician to provide comprehensive patient-centered care within a collaborative interdisciplinary team. This role emphasizes building long-term relationships with a manageable patient panel allowing providers to deliver meaningful holistic care and directly observe the impact of their work.
Key Responsibilities:
Conduct thorough in-person assessments and partner with the healthcare team to develop individualized care plans.
Coordinate patient discharges for acute short-term and long-term care needs.
Focus on delivering comprehensive high-quality care to a smaller number of patients per day.
Collaborate closely with other healthcare professionals to ensure continuity of care and optimal patient outcomes.
Candidate Requirements:
12 years of relevant clinical experience.
Board certification preferred in Internal Medicine Family Medicine Emergency Medicine or Geriatrics.
Eligibility to provide services under applicable healthcare programs.
Strong communication clinical judgment and organizational skills.
Commitment to patient-centered care and interdisciplinary teamwork.

This is a full-time position with general benefits.
