Overview
Catholic Charities is seeking a full-time Case Manager for our IRAP department (Immigration and Refugee Assistance Program) to help clients in Erie County at our Herkimer Office. This position is responsible for conducting health outreach for the Refugee Health Promotion program assisting with recruiting eligible individuals for wellness groups co- facilitating wellness groups and planning/executing health outreach events in collaboration with community partners. Health Outreach Case Manager will work directly with Medical Case Manager to coordinate services to promote health and mental health amongst newly arrived refugees and vulnerable populations.

Job Responsibilities
Assess client eligibility and enroll participants into the program
Conduct outreach within the community to identify eligible clients
Plan and co-facilitate monthly wellness groups
Plan bi-annual health outreach events in collaboration with community health partners
Collaborate with Medical Case Manager to assess client needs and identify gaps
As needed provide case management services (as a secondary provider)
Participate in internal program audits and file reviews
Maintain accurate documentation and input data into required systems.
Prepare reports and comply with all contract billing and documentation standards.
Build relationships and collaborate with community agencies.
Adhere to all federal and state regulations confidentiality and Catholic Charities compliance and ethics standards.
Perform other duties as assigned.
Minimum Qualifications
BA/BS degree or Associates degree with 2 years of relevant experience
Bilingual ability preferred (Swahili Arabic Burmese Kirundi Somali or Tigrinya)
Ability to work effectively in a multi-ethnic multi-cultural environment
Comfortable working with clients who have severe medical conditions
Strong organizational skills and excellent written verbal and interpersonal communication
Proficient in Microsoft Office (Excel and Word required); experience with data entry and reporting
Relevant office experience including accounts payable and ability to run reports
Ability to work independently and as part of a team
Valid NYS Drivers License and reliable vehicle for work-related duties
Evening and weekend availability as needed
