Under general supervision of the Client Advocacy Services Manager this is a professional level social casework and benefits counseling position. The Community Resources Coordinator works to implement advocacy/service programs for the elderly as well as adults with disabilities and their caregivers in Harford County including the Senior Care Program Ombudsman Program the Family Caregiver Support Program and the Maryland Access Point (MAP) should have basic computer skills including Word and Internet. Some evening and weekend work required.

Examples of Duties:
- Providing information to senior citizens and persons living with disabilities and their caregivers/families on community and institutional services
- Completing assessments of client and caregiver needs.
- Making appropriate referrals to a wide variety of service agencies.
- Assisting clients and their caregivers in completing application forms and following up to ensure clients/caregivers obtain services.
- Completing documentation in a timely manner in client files.
- Providing community education regarding services to senior citizen and caregiver organizations.
- Completing reports as required by the County and State for reporting purpose.
- Providing Case Management services for vulnerable older adults.
KNOWLEDGE SKILLS & ABILITIES:
- Thorough knowledge of older adults and persons with disabilities and their needs.
- Knowledge of relevant community resources in the health and welfare fields and resources available to support older adults and adults with disabilities and of major components of the long-term care industry and how to access to these services.
- Knowledge of social psychological and medical issues that affect the elderly.
- Knowledge and skill in the functions of social casework.
- Skill in counseling and interviewing techniques.
- Ability to work effectively with older adults adults with disabilities and their families and caregivers and with professionals and the public.
- Ability to communicate effectively both orally and in writing; to exercise professional judgment; to adapt to emergencies and to work independently.
- Ability to prepare statistical and narrative reports
- Ability to drive motor vehicles to climb stairs to bend stoop kneel or sit depending on work assignment.
- Worker is subject to inside and outside environmental conditions.
Years of Experience:
Two (2) years experience providing social casework counseling community health nursing (with Maryland license if in nursing) or a field closely related to aging services. One year of experience may be substituted with a Masters degree.
Education:
Bachelors degree in social work gerontology sociology psychology nursing counseling or human services-related field.
SAFETY:
Must comply with all Federal (OSHA) State of Maryland (MOSH) safety regulations and applicable Harford County safety policies procedures or manual as required for this position.
