Job Summary
Job Title: Registered Nurse (RN) Case Manager
Location: Syracuse NY 13210
Contract Duration: 13 Weeks
Shift: 9:00 AM 5:30 PM
Pay Rate: $53 $56/hour
Position Summary
Navitas Healthcare LLC is seeking an experienced Registered Nurse (RN) Case Manager with strong skills in Utilization Review Clinical Documentation Improvement (CDI) and MDS assessments. This role supports patient care coordination documentation accuracy and regulatory compliance within an acute care hospital environment. The ideal candidate is detail-oriented clinically strong and comfortable working in a fast-paced setting.
Qualifications
Active New York State RN License/Registration Required
BLS Certification (must be eligible for recertification) Required
Minimum 1.5 years of recent acute care hospital experience (within the past 2 years) Required
Experience in equal or higher-level hospitals or trauma centers Required for travelers
MDS (Minimum Data Set) experience Required
EPIC EMR experience Preferred
Strong communication assessment and critical-thinking skills
Key Responsibilities
Coordinate and manage individualized patient care plans
Conduct and document accurate MDS assessments
Support CDI initiatives to improve documentation quality
Perform utilization reviews to ensure appropriate level of care and regulatory compliance
Collaborate with physicians nurses social workers and interdisciplinary teams
Ensure documentation supports medical necessity and reimbursement standards
Monitor patient progress and assist with discharge planning
Maintain precise and timely documentation using EPIC EMR
Adhere to hospital policies procedures and regulatory requirements
