Job Summary:
The Registered Nurse will be responsible for providing skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Daily tasks include patient assessments medication administration treatment planning and collaborating with the interdisciplinary team. Strong communication and collaboration with patients providers and other members of the team are critical to this role.

Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Two years minimum of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in the state of New York
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Specialities in Behavioral Health
Special Considerations:
- Must be more than 75 mile radius
Scheduling:
- 3x12 hour shifts 1900-0730
- Every other weekend required
- On call not required
- Start date: 1/5/25
