Job Summary

Job Summary:

The Occupational Therapist provides skilled occupational therapy services to patients in their homes focusing on improving their ability to perform daily living activities. This role involves patient assessments treatment planning and collaboration with an interdisciplinary team to optimize patient outcomes. The therapist will travel to patients homes within a designated service area providing individualized care and support.
Location:Medford Oregon United States
Responsibilities:

- Conduct patient assessments to determine occupational therapy needs and develop appropriate treatment plans.
- Provide skilled occupational therapy interventions to improve patients functional abilities and independence.
- Educate patients and their families on therapeutic techniques and home exercise programs.
- Collaborate with physicians nurses and other healthcare professionals to coordinate patient care.
- Maintain accurate and timely documentation of patient assessments treatment plans and progress notes using EPIC/Oasis charting system.
- Manage a caseload of patients optimizing travel routes to maximize efficiency and productivity.
- Adhere to all facility policies and procedures including infection control and safety guidelines.
Required Skills & Certifications:

- Active Occupational Therapist license in the state of Oregon.
- Basic Life Support (BCLS) certification.
- Minimum of 1 year of skilled home health experience.
- Oasis experience.
- Valid drivers license and reliable transportation with appropriate insurance.
Preferred Skills & Certifications:

- Experience with EPIC/Oasis charting systems.
Special Considerations:

- Traveler must have their own transportation.
- The facility requires primary series COVID vaccination in addition to one booster.
- The facility can call off UP TO one shift per pay period (every two weeks).
- Mileage reimbursement at IRS standard rates.
- May visit up to 6 patients per day; 30 productivity points standard.
- Occasional maximum radius of 50 miles.
- RTO restrictions: Confirm with hiring manager.
Scheduling:
- 8-hour day shifts.
- No 4x10s or 3x12s shifts are accommodated.
- No weekend requirement.
- No on-call requirement.
- No block scheduling is accommodated.
