Registered Nurse Case Manager

Responsibilities:

1. Conduct comprehensive assessments of patient needs and develop individualized plans of care.
2. Coordinate and manage a caseload of home health patients including skilled nursing physical therapy occupational therapy and speech-language pathology.
3. Obtain and review physician orders referrals and medical records.
4. Educate patients and families on disease management medication administration and self-care techniques.
5. Monitor patient progress identify potential complications and adjust care plans as needed.
6. Ensure compliance with all regulatory and accreditation standards.
7. Maintain accurate and timely documentation in electronic health records.
8. Collaborate effectively with physicians nurses therapists and other members of the healthcare team.

The Case Manager oversees a limited number of patients instead of treating as many as possible in a short time. Our Case Management team focuses on QUALITY over QUANTITY.

Qualifications:

1. Current and valid Registered Nurse (RN) license in the state of Iowa.
2. Current CPR Card.
3. Valid drivers license reliable vehicle and auto insurance.
4. 1 year of experience in home health or a related setting (preferred).
5. Strong organizational time management and problem-solving skills.
6. Excellent communication and interpersonal skills both written and verbal.
7. Proficiency in electronic health records (EHR) systems.
8. Ability to work independently and as part of a virtual team.
9. Reliable high-speed internet access and a dedicated workspace at home.
