Job Summary:
The London Borough of Hackney is seeking two Reviewing Officers to support the Adults Health & Integration Directorate within the Adults Commissioning Review Team.
The postholders will carry out scheduled reviews of care and support plans for adults receiving services under the Care Act 2014 ensuring that packages of care remain appropriate outcome-focused and deliver value for money.
Working closely with social workers providers and commissioning colleagues the Reviewing Officers will assess service quality engage with service users identify changes in need and contribute to continuous improvement in adult social care provision.
This role requires strong communication skills good organisational ability and experience within adult social care settings.
Key Duties/Accountabilities (Sample):
Conduct scheduled reviews of care and support plans for adults receiving services.
Ensure services remain appropriate outcome-focused and meet individual needs.
Engage with service users carers and providers to gather feedback and assess satisfaction levels.
Monitor the delivery and quality of commissioned services.
Identify any changes in need and escalate cases requiring reassessment to qualified social workers.
Maintain accurate and timely case notes on the Councils case management system.
Ensure compliance with the Care Act 2014 safeguarding principles and Hackney Council policies.
Work collaboratively with social workers commissioning officers and service providers.
Provide data insights and information to support commissioning decisions and performance monitoring.
Skills/Experience:
Experience working in adult social care or a similar setting.
Strong communication and interpersonal skills with the ability to engage sensitively with adults carers and professionals.
Proven experience reviewing or monitoring care and support plans.
Understanding of the Care Act 2014 person-centred care and safeguarding principles.
Ability to manage a caseload independently and work to deadlines.
Knowledge of commissioning processes and quality assurance in adult social care.
IT literate with experience using case management or social care systems.
Ability to gather analyse and report on information accurately.
Additional Information:
Directorate: HAC Adults Health & Integration (Adults Commissioning Review Team).
Location: Hackney London.
Hours: 36 per week.
Hybrid/office expectations as per Hackney Council working arrangements.
