Schedule: Full-Time - 8:30 AM 5:00 PM with one required late day per week (11:00 AM 7:30 PM)
What Youll Do
As a Case Manager youll play a vital role in guiding patients through their recovery journey by:
- Providing case management and care coordination services for clients in the facility.
- Coordinating continuum of care activities to ensure optimal resource utilization and compliance with referral sources.
- Conducting psychosocial assessments and facilitating individual family and group sessions.
- Managing treatment and discharge planning activities.
- Developing implementing and evaluating personalized care plans.
- Collecting and correlating clinical data for the treatment team and insurance providers.
- Serving as a liaison between the treatment team and external agencies to ensure seamless care.
What Were Looking For
- Strong organizational and communication skills.
- Ability to work collaboratively with a multidisciplinary team with a minimum case load of 30 clients
- Minimum of 3 years Experience in behavioral health or substance use disorder preferred.
- Minimum of a bachelors degree in social work Counseling Criminal Justice or a related Human Services field.
