Qualifications

Active New York State RN License
Primary Source Verification from NYSED
AHA BLS Certification
Associate Degree in Nursing (ASN/ADN) from an accredited program
Minimum 2 years of acute care experience in one or more of the following: Med-Surg Maternal & Child Care Critical Care Pediatrics
Current adult care home health experience
Ability to work alternating weekends

Responsibilities

Provide high-quality home health nursing care to adult patients.
Conduct patient assessments develop care plans and evaluate progress.
Manage medication administration wound care and chronic condition monitoring.
Coordinate with interdisciplinary teams to ensure comprehensive patient care.
Educate patients and families on treatment plans safety measures and health maintenance.
Document all care according to regulatory and agency standards.
Support on-call or weekend rotations as scheduled.
