Position Summary Lead Caregiver
We are seeking an experienced adaptable Lead Caregiver (or CNA) to join our team in a dynamic leadership-track role. This position is ideal for someone who:
The Lead Caregiver is our go-to team member for launching new cases covering urgent shifts supporting higher-acuity clients and providing field support to help develop our caregiver team.
This role is the first step into leadership at The Next Chapter with an advancement path toward Assistant Care Coordinator.

High-acuity cases requiring skilled patient-centered care
Experience in ALL of the above areas is required for this position.
Key Responsibilities
Standard Caregiver Responsibilities (all still apply):
Daily Living Support
Assist with ADLs including bathing grooming dressing toileting feeding and mobility.
Specialized / High-Acuity Care
Provide advanced support for clients who are bed-bound or require mobility equipment (e.g. Hoyer lifts).
Deliver compassionate dementia and Alzheimers care.
Provide calm grounded support for hospice and end-of-life cases.
Companionship & Engagement
Offer meaningful social interaction and emotional support.
Housekeeping & Meal Preparation
Perform light housekeeping and prepare nutritious meals to maintain a safe clean and comfortable environment.
Medication & Routine Assistance
Provide medication reminders and support consistent daily routines.
Errands & Transportation
Transport clients to appointments and activities as needed.
A reliable personal vehicle valid drivers license and current auto insurance are required.
Safety & Supervision
Follow individualized care plans and observe for changes in condition.
Additional Lead Caregiver Responsibilities
Case Launch & Immediate Support
Serve as the first caregiver on new cases to establish routines expectations and rapport.
Provide urgent coverage for call-outs and last-minute needs M-F.
Training & Field Support
Mentor caregivers in the field reinforce best practices and build confidence.
Assist leadership with hands-on quality assurance at client homes.
Leadership Communication
Communicate proactively with TNC leadership regarding client observations care plan needs and family dynamics.
Identify risks challenges or needed supplies promptly.
Complex Case Handling
Remain steady patient and solution-oriented in challenging family or care scenarios.
Community Engagement
Participate in occasional volunteer or community events as a representative of TNC.
Requirements
Experience
2 years caregiving experience in a home care setting required
5 years strongly preferred
Experience with dementia hospice and high-acuity clients required
CNA preferred but not required
Screening & Documentation
Background check drug screening DMV check and negative TB test
Fingerprint Clearance Card preferred
Certifications
Current First Aid & CPR
Caregiving Certification or CNA license required before client assignment
(Certification support available for highly experienced applicants)
Transportation
Reliable personal vehicle active auto insurance valid drivers license
Must be willing to cover our full service area (generally within 30 minutes from Chandler; occasional farther travel may be needed)
Availability Requirements
This role requires full MondayFriday availability for any shift (days evenings or overnights).
Although scheduling needs can change based on client acuity and urgent coverage we aim to build schedules that are fair respectful and minimize unnecessary disruption (days & nights within the same week) whenever possible.
Compensation & Benefits
Compensation
Competitive hourly rate $20-25/hr based on experience and acuity
Guaranteed 30 hours/week minimum regardless of census
Most weeks expected to reach 40 hours
Additional shifts (including weekends) may be offered based on business needs
