LPN Full Time Nights
Requirements: Active NY LPN License BLS (AHA or RQI) 1 year LPN experience weekend/holiday rotation required.
Complete Description (Condensed):
This Night-Shift LPN role provides direct and indirect nursing care to adult and geriatric residents in a long-term care setting. The LPN is responsible for collecting and reporting resident data contributing to age-appropriate care plans and collaborating closely with the RN and interdisciplinary team. Duties include evaluating resident responses to interventions recommending updates to the plan of care maintaining clinical competency and promoting a safe culturally sensitive environment. Position involves mentoring/orienting staff supporting resident/family communication and adhering to established policies and infection control standards. Occupational exposure to blood-borne pathogens may occur.
Job Duties (Summarized):
Collect and report physical/psychosocial data to RN/IDT
Assist in developing age-appropriate culturally relevant care plans
Recommend revisions based on resident outcomes
Support safe cost-effective care delivery
Maintain clinical competency and participate in continuing education
Collaborate with residents families and the care team
Mentor/orient new staff as directed
Follow RN supervision and all LTC facility policies
Required Skills/Experience:
NY LPN Licensure Required
