Job Description

Job Summary

This position supports the Department of Psychiatry under the direction of the Manager of Operations for Collaborative Care (CC) the individual is responsible for planning implementing and evaluating collaborative care initiatives.

This key role involves:

  • Documenting treatment plans and communicating patient needs across caregivers and providers.

  • Providing patient care management services including outreach and resource allocation.

  • Maintaining a patient registry and tracking health outcomes.

  • Coordinating multidisciplinary teams and managing project materials.

  • Facilitating data collection and reporting to demonstrate impact.

  • Outreach to patients on an assigned caseload

  • Patient Registration and appointment scheduling

  • Completing intake forms and screening tools

  • Coordinating with the patients PCP and mental health providers. Tracks and records referrals and evaluations flowing through the program.

  • Facilitating patient engagement and follow-up care

  • Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments as indicated.

  • Supports the submission of reports to external entities including funders Office of Sponsored Projects Foundation

  • Coordinates staff meetings creates agendas keeps minutes and action items.

  • Provides administrative support to the CC providers/faculty.

  • Develops and maintains CC dashboards.

  • Supports the departments (CC) newsletters -works with staff members to plan and develop content including the identification of staff and/or or programs to be featured assembles and organizes related content and coordinates distribution.

  • Prepares composes and distributes routine correspondence memoranda meeting minutes and other documents for CC programs.

  • Assumes other administrative responsibilities as may be assigned from time to time by the leadership of the Department/CC

BHCM specific functions

  • Systematically track treatment response and monitor(patients for changes in clinical symptoms and treatment side effects or complications.

  • Under the direction of the CC Medical Director will supporting psychotropic medication management as prescribed by medical providers focusing on treatment adherence monitoring side effects and effectiveness of treatment.

  • Provide patient education

  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation problem-solving treatment motivational interviewing or other treatments as appropriate and as directed by CC clinical leadership.

  • Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicating resulting treatment recommendations to the patient/patients medical provider.

Minimum Requirements:

  • Bachelors degree required.

  • Two (2) years of experience in Human Resources Organizational Development Healthcare Administration nonprofit management or related field preferred but not required.

  • Excellent writing and interpersonal communication skills required; some grant writing experience preferred.

  • Proficient with Microsoft computer software applications working knowledge of content management and publishing software and database technologies.

  • Ability to work independently with limited supervision.

  • Organizational skills: Ability to establish priorities and react promptly to a large variety of requests and needs sometimes in stressful situations.

  • High level of attention to detail; ability to multi-task.

Shift: Monday - Friday / 8am-4pm

Pay Range

$59280.00-$96928.00 Salary
