Job Summary:
The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Key responsibilities include patient assessments medication administration treatment planning and interdisciplinary team collaboration. Strong communication and collaboration with patients providers and team members are essential.
Location:Napa California United States
Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments and develop treatment plans.
- Administer medications and treatments as prescribed.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Communicate effectively with patients providers and other team members.
- Perform case management duties.
- Provide hospice care as needed.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Bachelors degree in Nursing or related field.
- Minimum of 2 years of hospital home health or hospice care experience.
- Clinical Assessment skills.
- Medication Administration skills.
- Treatment Planning skills.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Drivers license car insurance and automobile for field work
Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room.
Special Considerations:
- Must be willing to float between like units.
- Weekend work is required.
- On-call may be required.
- RTO must be included at the time of submission.
Scheduling:
- Variable start times between 6:30 AM and 8:30 PM.
- Schedule determined per manager discretion for block scheduling.
