Job Summary:
The Registered Nurse will provide skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration with patients providers and other team members are critical.

Location:Napa California United States
Responsibilities:
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Collaborate with patients providers and other team members.
- Float between Acute Rehab and other Inpatient floors.
- Provide skilled nursing care.
- Perform patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Provide case management.
- Provide hospice care.
- Collaborate with patients providers and other team members.
- Float between Acute Rehab and other Inpatient floors.
Required Skills & Certifications:
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum of 2 years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Active Registered Nurse license in California
- Bachelors degree in Nursing or related field
- BLS certification

- Minimum of 2 years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment

Preferred Skills & Certifications:
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room
- Specialities in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room
Special Considerations:
- Must be willing to work one weekend day (typically Sunday) every other weekend.
- Facility is able to call off once per two week pay period.
- Open to accommodating block schedule per manager discretion.
- Must care for Adolescents Adults and Geriatrics.
