Job Summary:
The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. The role requires strong communication and collaboration skills with patients providers and team members. Acute rehab experience is preferred but candidates willing to float there with proper orientation will be considered.

Responsibilities:
- Provide skilled nursing care to patients.
- Conduct patient assessments.
- Administer medications.
- Develop and implement treatment plans.
- Collaborate with the interdisciplinary team.
- Communicate effectively with patients providers and team members.
- Case management.
- Hospice care.
Required Skills & Certifications:
- Active Registered Nurse license in the state of working.
- Clinical Assessment Medication Administration and Treatment Planning skills.
- Minimum 3 years of experience.
- Interdisciplinary Team Collaboration skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Bachelors degree in Nursing or related field.
- Drivers license car insurance and automobile for field work.
Preferred Skills & Certifications:
- Acute rehab experience.
Special Considerations:
- Must be willing to float between like units.
- RTO must be included at the time of submission.
Scheduling:
- Schedule could be Tuesday through Saturday or Sunday through Thursday.
- Specific shift details not provided.
- Block scheduling is per manager discretion.
