Reports to: Manager Program Services
Hours: 37.5 Hours/Week
Salary: $27.51 (start rate determined in accordance with collective agreement requirements and is non negotiable)
Shifts: Day Shifts (every other Saturday or Sunday)
Status: Permanent Full Time

RESPONSIBILITIES:
- Provide social educational and recreational programming to create a positive and engaging experience for clients
- Plan and organize group activities with other team members based on clients cognitive and physical capabilities
- Program preparation including purchase of program supplies producing of program flyers activity calendar and etc.
- Maintaining a safe environment for program operation including activity area cleanliness equipment maintenance IPAC compliance etc.
- Transporting and/or escorting program participants to and from programs with the support of personal support workers
- Documents clients abilities and records participation
- Evaluate the success of events and activities gather feedback from clients and make improvements for future activities
- Other duties assigned as required.
Qualifications :
- Post-secondary education in social and health services at least at a Community College Level with work experience and knowledge in elderly social service or equivalent.
- Knowledge and training in GPA PIECES and Restorative Care an asset
- At least 1 year of working experience in community setting
- Sensitivity to recognize the aging and physical needs of elderly clients
- Good organizational and time management skills
- Ability to work independently and as part of a team
- Excellent organizational communication and interpersonal skills.
- Proficient computer skills in Microsoft Office
- Current certification in CPR
- A valid vulnerable sector police record check completed within one year
- Good attendance record is required
- Current UHN employees must have successfully completed their probationary period have a good employee record along with satisfactory attendance in accordance with UHNs attendance management program to be eligible for consideration.
