In this role youll evaluate physical and cognitive function design and implement individualized treatment plans and employ evidence-based interventions to maximize functional outcomes. Core responsibilities include comprehensive assessments goal setting and progress documentation; collaboration with therapists nurses physicians and social workers to coordinate care across inpatient outpatient rehab and home-care settings; modification of activities for safety balance motor planning and adaptive equipment; and discharge planning with family or caregivers to ensure sustained gains. Youll monitor progress adjust plans as needed and advocate for patient access to resources that support long-term independence.
