Job Summary

The Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) provides comprehensive primary and preventive medical care to geriatric patients in community-based settings including patient homes and nursing facilities. Care is delivered in collaboration with a CHMG physician with a focus on optimizing health independence and quality of life for older adults. This role requires travel to patient residences and long-term care facilities.

30-hour role

Education

Masters degree in Advanced Practice Nursing or completion of Physician Assistant program.

Certification Registration & Licensure

  • NH APRN or Physician Assistant license

  • Current DEA license

  • National certification in specialty area preferred

Experience

  • Minimum of 23 years of clinical experience in geriatrics or primary care with older adults

  • Experience in home visits or nursing facility care strongly preferred

  • New graduates will not be considered for this role

Responsibilities

  • Obtain detailed patient histories perform physical examinations and develop appropriate differential diagnoses for geriatric patients

  • Order interpret and follow up on diagnostic tests

  • Diagnose and manage acute and chronic conditions within the scope of practice

  • Prescribe and manage medications according to clinical guidelines and practice standards

  • Provide individualized patient education focused on preventive care chronic disease management and overall wellness for older adults

  • Maintain thorough timely documentation of patient encounters in the medical record

  • Collaborate closely with CHMG physicians nursing staff and other care team members to ensure continuity of care
