Salary Range: $50k - $55k

Neighborhood Self Help by Older Persons Projects (SHOPP)is a dynamic and innovative non-profit organization that supports older adults caregivers and individuals with disabilities. SHOPP offers 12 essential programs including CCNP (Community Care Network Program) 4 Older Adult Centers (Casa Boricua Leon Soundview and Guess) NORC (Naturally Occurring Retirement Communities) Pop Up Café Case Management with Friendly Visiting Elder Justice Caregiver Support Program and NY Connects. SHOPPsmissionis to enhance the strength and resilience of individuals regardless of age or ability through innovative programs and support services.

NYConnectsis the trusted place to go for free unbiasedinformationaboutlong-termservices and supports in NYS for people of all ages or with any type of disability. Long-term services and supportsincludemedicalandnon-medical services a person needs toimproveor maintain health and independence. The NY Connects program may provide services in a nursing hometheindividuals home or other community-based settings. Our goal is to help individualsliveindependently while meeting their medicalsocialandfunctional needs that arise fromagingor disability.

TheNY Connects Information & Assistance (I&A) Specialistprimarily delivers information and assistance onlongtermcareservicesand supports to individuals familiescaregiversprofessionals and Bronx communities who arein need of accessing and navigating through complex systems.

Minimum Qualifications:

The position requires an Associates Degree from an accredited college in Social Work Human Services Psychology Gerontology or equivalent and a minimum of two years of experience in long-term services and supports health care developmental disabilities mental health or a related area of human services.

TheNY Connects I&A Specialistis specifically responsible for the followingtasks:

• Provides Information and Assistancetoconsumers through the phone e-mail orin-person.
• Provideunbiasedinformation and assistanceonalllong-termcare services/supportsconductsgeneralscreening of consumers to determine needsmaylink consumerswithlong-termcare services and supportsand may provide follow-upwith consumersto ensurethatconnections to serviceswere made.
• Acquires and maintains a thorough knowledgeofall locallong-termcareprograms as applicableto olderadults andindividuals with disabilities.
• Maintain accurateconsumerl&A records.
• Build connections and correspondtoemails and phone callsin a timelymanner.
• Educate communitiesandbuildlinkagesand contacts for the program.
• Maintain greatinterpersonaland communication the extra mile to assist participantsinmeeting the programs goal.
• Advocatereferand assist with application Delivered Meals.
• Complywiththe New York State NYConnectsStandards when interactingwithallconsumersand performing otherjob also comply with SHOPPPersonnelPolicy.
• Coordinate regular Public Education/Outreach events in thecommunityto generatereferrals to the program.
• Follow outreach plan to promote the program and its services.
• Communicate and coordinate outreach events with internal departments.
• Promote SHOPPservicesand participateincommunity fairs andevents (indoorsand outdoors)through face-to-face meetings or virtually due to the pandemic.
• Performothertasks as assignedbythe Program Director.
