Job Summary
Location: Oakdale
We are seeking a dedicated and experienced RN / Patient Care Manager in Home Health to join our team. This role is central to ensuring our patients receive high-quality compassionate care in the comfort of their homes. The ideal candidate will possess strong leadership skills clinical expertise and a commitment to improving patient outcomes.
Key Responsibilities:
- Oversee the day-to-day clinical operations of the home health team ensuring compliance with healthcare regulations and standards.
- Coordinate patient care plans working closely with physicians therapists and other healthcare providers to deliver comprehensive care.
- Supervise and support nursing staff providing guidance and assistance as needed to maintain high standards of care.
- Conduct regular evaluations of patient care services implementing improvements to enhance service delivery.
- Facilitate communication between patients families and healthcare providers to ensure clear understanding of care plans and expectations.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
Qualifications:
- Registered Nurse (RN) with a valid nursing license in the state of operation.
- Minimum of 3 years of nursing experience preferably in a home health setting.
- Prior experience in a managerial or supervisory role is highly desirable.
- Strong organizational and communication skills.
- Ability to work independently and manage multiple priorities effectively.
- Commitment to maintaining the highest standards of patient care and confidentiality.
