Job Summary
In accordance with the Vision Purpose and Values and strategic direction of the Vancouver Island Health Authority patient safety is a priority and a responsibility shared by everyone at VIHA; as such the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Reporting to the Manager or designate provides social work intake services and counselling to clients to assist in meeting the psychosocial needs of clients and to enable clients to utilize medical care in order to achieve their optimal level of health Prepares intake assessments and participates in multi-disciplinary teamwork liaises with and obtains resources from community social health and welfare agencies. Clinical guidance/instruction is available from the Senior Therapist.
QUALIFICATIONS:
Education Training And Experience
Degree in Social full registration with the BC College of Social Workers . Familiarity/experience with application of Family-Centred practice. Good understanding of community health welfare and social agencies. Basic knowledge and experience of intake process and multi-disciplinary teamwork.
