Position Summary
A managed care organization serving Kansas Medicaid members is seeking an experienced and compassionate nurse (RN or LPN) to join its Care Management team in a fully remote role. The Admissions Coordinator will manage and facilitate admissions into long-term care skilled nursing facilities (SNFs) and rehabilitation programs for members who require post-acute or extended care services. Candidates should have experience in home health senior living intake long-term care admissions or post-acute placement along with a solid understanding of Kansas Medicaid (KanCare) processes and care transitions.
Key Responsibilities
Coordinate and oversee admissions and transitions of care for members entering long-term care skilled nursing facilities or rehabilitation settings across Kansas.
Review clinical documentation and assessments to support appropriate placement based on member needs and Kansas Medicaid (KanCare) criteria.
Serve as the primary liaison between the health plan SNF admission teams hospital discharge planners and providers to ensure timely appropriate placements.
Manage prior authorization workflows verify eligibility and support care approval processes within RN/LPN scope.
Collaborate closely with Utilization Management Case Management and interdisciplinary teams to ensure safe efficient transitions of care.
Provide education to members and caregivers regarding benefits services and expectations during transitions.
Maintain accurate documentation in EMR and care coordination systems.
Ensure compliance with Kansas regulations KanCare standards HIPAA requirements and organizational quality benchmarks.
Qualifications
Active unrestricted Kansas RN or LPN license (required).
Minimum of 23 years of experience in home health long-term care rehabilitation hospital admissions or intake/placement coordination.
Knowledge of Kansas Medicaid/KanCare systems authorizations and level-of-care guidelines.
Strong communication problem-solving and documentation skills.
Proficiency with EMRs and care coordination technology.
Ability to work independently in a remote fast-paced environment.
Preferred Experience
Managed care insurance UM/CM or health plan experience.
Background in discharge planning post-acute transitions or SNF-level placement.
Bilingual English/Spanish or English/Haitian Creole is helpful but not required.
