Job Summary

JOB SUMMARY:

Social Worker will provide direct services to individuals with substance use disorders within an OASAS 822 licensed outpatient program. Services will be delivered both in-person and via telehealth as well as in off-site community locations as needed to support engagement and retention. The clinician will be responsible for conducting assessments providing individual and group counseling and coordinating care across ICLs integrated behavioral health continuum while adhering to trauma-informed culturally responsive and evidence-based practices.

ESSENTIAL JOB FUNCTIONS:

  • Conduct comprehensive assessments including biopsychosocial SUD screenings and treatment planning in accordance with 822 regulations
  • Deliver individual group and family counseling services both in-person and via HIPAA-compliant telehealth platforms
  • Provide off-site services in community-based settings to improve access and engagement
  • Develop and update person-centered treatment plans that reflect client goals and regulatory requirements
  • Utilize evidence-based modalities such as Motivational Interviewing (MI) Cognitive Behavioral Therapy (CBT) and Harm Reduction approaches
  • Collaborate with internal and external providers to support coordinated care (mental health medical housing etc.)
  • Maintain timely and accurate clinical documentation in the electronic health record in compliance with OASAS Medicaid and agency standards
  • Participate in clinical case reviews team meetings trainings and quality improvement activities
  • Support clients in navigating recovery-oriented resources and re-entry into the community
  • Uphold ethical standards and integrate cultural humility and anti-oppressive practice in all services
  • Other job duties as required.

QUALIFICATIONS AND EXPERIENCE:

Masters degree in social work (MSW) from an accredited institution. NYS LMSW or LMHC license required. Bilingual candidates and those with additional certification such as a CASAC-T or CASAC credentials are strongly encouraged to apply.
