Program Summary
The Community Health and Wellness sector provides services that support the physical behavioral and emotional health and wellbeing of individuals and communities. Our staff are certified Traditional Health Workers (such as Community Health Workers (CHW) Peer Support Specialists (PSS) and Doulas) who offer direct services such as healthcare and systems navigation health insurance enrollment physical and emotional health support adjustment support mental health resource navigation and client-directed case management. We also engage in community outreach and physical and emotional health education via community workshops psycho-education support groups and by hosting monthly health clinics that provide dental services health services and community resource information.


Position Summary
The HRSN Housing Case Manager will provide housing and resource navigation services to eligible Oregon Health Plan (OHP) members. The HRSN Housing Case Manager will conduct eligibility screenings assist clients with obtaining required documentation and completing HRSN requests coordinate delivery of authorized rent and utility assistance and other benefits support clients in developing and implementing housing plans support clients in maintaining OHP enrollment and assist clients with accessing both immediate resources and long-term programs and services. The case manager will also engage in outreach and community engagement to ensure communities and clients have knowledge of and access to these benefits. Due to the structure of the HRSN benefits this role will require thorough and timely completion of all documentation and administrative tasks. This position reports to the Community Health Coordinator.


Essential Functions

  • Support clients in completing HRSN request forms including gathering all necessary documentation and following up on submitted requests as needed.
  • Work with vendors including landlords and utility companies to obtain the documentation required for provision of benefits within authorization dates and spending limits.
  • Coordinate the delivery of rent and utility payments and other housing benefits including storage fees home modifications and home remediations.
  • Provide high quality housing case management including support in developing housing plans applying for housing addressing barriers to retaining housing communicating effectively with property owners/managers complying with a lease managing a household budget and developing other independent living skills.
  • Help clients secure and maintain assistance and services including entitlement programs such as TANF SNAP and WIC as well as other federal state and local programs.
  • Connect clients to settings where basic needs can be met such as access to showers laundry shelter and food.
  • Support clients in accessing medical peer social education legal imminent eviction prevention and other services.
  • Make referrals as needed to both internal IRCO programs or external partners to meet clients needs as they are expressed.
  • Conduct in-person meetings with clients either at an IRCO office or at the clients home as needed
  • Complete documentation in the electronic case management system and client files as required including service planning releases of information case notes and precise time tracking.
  • Maintain a high level of confidentiality.
  • Engage in outreach in the community to provide knowledge of and access to HRSN benefits for potentially eligible clients.
  • Participate in case reviews with other team members and coordinate connections with community resources as necessary.
  • Participate in staff meetings training committees and other Community Health and Wellness activities to support the sector and agencys goals.
  • Other duties and responsibilities may be assigned.

Secondary Functions

  • Experience working in rental and/or utility assistance programs
  • Experience with case management and resource navigation
    Experience working with or understand refugee and immigrant communities
  • Ability to discuss sensitive topics (health related conditions and social determinants of health financial or food insecurity) with care and respect
  • Demonstrated ability to work effectively independently and as part of a team
  • Strong computer skills including proficiency in the use of Microsoft programs and applications and understanding of working with a case management database
  • Strong written and verbal communication skills
  • Ability to function in a positive manner in a demanding work environment to demonstrate a high degree of flexibility to respond to priorities and schedules that change frequently and to meet commitments
  • Must be able to maintain strict confidentiality and follow HIPAA policies and procedures.


Requirements

Education & Experience

MINIMUM JOB SPECIFIC QUALIFICATIONS:

  • High School Diploma or GED
  • Minimum 1 year of work experience


PREFERRED QUALIFICATIONS:

  • Certification as Community Health Worker in the State of OregonSM1 ML2 TT3
  • Bilingual in English and at least one relevant language for the communities of Africa Greater Middle East Pacific Islander Asian or Slavic/Eastern European.
