Home Health RN Registered Nurse Empath Home Health - Port Charlotte North Port Venice

Empath Home Health a member of Empath Health is seeking a compassionateHome Health RN Registered Nurseto provide skilled nursing care for patients in their homes throughout Charlotte county (Port Charlotte North Port Venice Englewood Rotonda West Arcadia and surrounding areas). In this full-time role youll deliver individualized care build trusted patient relationships and help people manage their health with dignity and independence.

Locations: Home health visits in Charlotte county including: Port Charlotte North Port Venice Englewood Rotonda West Arcadia and surrounding areas

Schedule: Full-Time M-F 40 hours. Required weekend/holiday on-call rotations. PRN is available if desired!

What Youll Do

• Perform comprehensive patient assessments including OASIS assessments at required intervals
• Develop initiate and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes self-care techniques and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless holistic care

What Youll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment critical thinking and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
