PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors their family members and caregivers face many issues including transportation to appointments management of medications coordination of medical care from different specialists lack of social interaction and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care social and recreational activities and other senior care services.

POSITION SUMMARY: Under the direction of the Program Manager the Center Scheduler is responsible for the scheduling and coordination of in-house and outside appointments for On Lok participants.


DUTIES / RESPONSIBILITIES:

  1. Generate specialty referral and schedule both in-house and outside medical appointments.
  2. Provide all necessary documents required for medical and specialist appointments.
  3. Cancel/reschedule medical and specialist appointments and inform the participants and families.
  4. Enter and track all in-house and outside appointments in PaceLink Scheduler.
  5. Complete paperwork for all medical appointments.
  6. Complete appointments for audiology podiatry dental optometry and other specialty referrals in specialists and outside appointment in PaceLink Scheduler.
  7. Generate referral forms for visiting Primary Care Physician (PCP) and enter in PaceLink.
  8. Send all documents to PCP after appointments are completed.
  9. Obtain and keep track of diagnostic requests and billing forms as needed.
  10. Prepare podiatry forms for podiatry visits.
  11. Communicate effectively (written and verbal) with providers participants family and team members.
  12. Update PaceLink Scheduler for changes related to the appointment.
  13. Prepare and maintain monthly calendar for specialists in NextGen PM (NextGen Practice Management software).
  14. Provide coverage for other scheduler as needed.
  15. Call facilities to obtain test results if not reported to clinic in a timely manner.
  16. Prepare paperwork pertaining to meals and transportation as related to appointments.
  17. Coordinate services pertaining to appointments such as transportation meals translation and escorts.
  18. All other reasonably related duties as assigned.

QUALIFICATIONS (knowledge skills abilities)

  1. Minimum two years of experience in a healthcare setting preferably with the elderly.
  2. Knowledge of healthcare and proven experience in medical billing and scheduling from diverse backgrounds and job classifications.
  3. Demonstrated excellent communication skills both written and verbal.
  4. Proficient computer skills required.
  5. Flexibility initiative and willingness to participate in all aspects of the organization.
  6. Bilingual ability may be required for specific assignments.

Your final compensation offer will be determined based on factors such as skills experience education and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
