Job Summary

3 months contract with local authority

The role involves reporting to an Occupational Therapist within the Reablement team focusing on conducting strengths-based occupational therapy assessments and reviews for individuals using occupational therapy equipment and domiciliary care. The position requires working under relevant legislation such as the Care Act and the Housing Grants Construction and Regeneration Act to minimize handoffs and ensure environmental changes that support independence are accurately reflected in care provision. Collaborating with service users carers and various professionals the role aims to deliver occupational therapy interventions that enhance independence and wellbeing. Accurate documentation of contact and case notes on IT systems is also essential.

Responsibilities:

  • Conduct comprehensive occupational therapy assessments and interventions for residents with substantial needs.
  • Work within legislative frameworks and adhere to local organizational and national policies.
  • Present clear specifications for environmental adaptations for disabled residents.
  • Identify and manage risks related to specialist equipment in home environments.
  • Perform mobility assessments and ensure eligibility for concessionary travel.
  • Optimize the use of resources including time and funding.
  • Collaborate with service users carers and professionals to maintain high service standards.
  • Provide information advice and set up TeleCare support as needed.
  • Recognize vulnerable individuals and ensure timely support and interventions.
  • Promote activities and services that enhance health and wellbeing.
  • Ensure specialist equipment improves independence or ease of care effectively.
  • Maintain accurate and objective reports and IT records.
  • Adhere to confidentiality and data protection guidelines.
  • Contribute to a supportive and professional Reablement Team environment.
  • Engage with colleagues to deliver cohesive and effective services.
  • Share expertise and participate in case discussions and peer support.
  • Foster a culture of learning and continuous improvement.
  • Participate in team meetings training and reflective practice sessions.
  • Maintain a positive and solution-focused team dynamic.


Requirements


  • Accredited Trusted Assessor Qualification.
  • Disclosure and Barring Service (DBS) clearance.
  • Strong clinical reasoning and decision-making skills.
  • Proficient in database recording and IT systems for case management.
  • Effective communication skills both written and verbal.
  • Ability to work independently manage workload and meet deadlines.
  • Professional demeanor that instills confidence among stakeholders.
  • Commitment to continuous professional development.
  • Knowledge of the Care Act 2014 and related legislation.
  • Understanding of TeleCare assistive technology and strength-based person-centered approaches.
  • Awareness of the needs of individuals with physical and sensory disabilities.
  • Experience in conducting effective OT assessments and promoting independence.
  • Experience in collaborating with service users carers and other service providers.
