An opportunity exists to join the clinical team at Seymour:Permanent Part time 0.8 FTE per fortnight.

The suitable applicant with need to demonstrate strong clinical & assessment skills ability to work collaboratively with external stakeholders & community partners.

A Community clinician provides a critical role in the active coordination of care for people requiring further follow up & linkages within the community.

The clinician will be required to communicate with referrers & treating teams as required & ensure that service continuity is maintained for consumers supports & family receiving assistance within mental health services.
