SUMMARY DESCRIPTION OF WORK

Care Team Specialist serves as the behind-the-scenes point person who ensures consistent communication among a clients care team both within the organization and with external providers to support optimal care. They help connect clients to additional resources or programs as needed and are responsible for completing the documentation required for the Certified Community Behavioral Health Clinic (CCBHC).

Compensation: between $19.74 and $23.54 per hour (wage is based upon total years of relevant experience).

Work Schedule:37.5 hours per week

MINIMUM QUALIFICATIONS

  • Bachelors degree in psychology social services sociology or other related fields desired or comparable work experience preferred
  • One year experience in health and human services or related field

Essential Duties and Responsibilities

  • Document client contact Release of Information consents for treatment and all required agency documentation.
  • Ensure completion of agency-wide assessments (e.g. PHQ-9 DLA-20 Columbia Suicidality Rating Scale if applicable).
  • Conduct and document assessments at admission and ongoing intervals identifying client strengths abilities needs and goals.
  • Develop individualized care plans including coordination crisis intervention and self-care strategies in collaboration with the client.
  • Advocate for client needs and ensure consistent engagement in services.
  • Connect clients to appropriate resources such as physical health behavioral health/substance use treatment housing education employment and social services.
  • Maintain monthly contact with clients through face-to-face meetings or telehealth.
  • Coordinate with the clients support network including family healthcare providers behavioral health professionals social service agencies and legal representatives.
  • Facilitate communication among providers organize case conferences and utilize team expertise to support client care.
    *Create maintain and share as appropriate a dynamic self-care plan.
  • * Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agencys ongoing fundraising and friend-raising efforts.
  • * Essential functions of this job are to be performed on company physical work site or designated workspace.
