Job Summary

Summary

Position Summary: The Hospice and Palliative Care Advanced Practice Professional(APP) is an APPwho focuses on the care of patients with life threatening illnesses. The APP performs to the full scope of his/her license demonstrating competent clinical knowledge and clinical practice education collaboration consultation and research. The APPs overarching efforts are focused on relieving suffering enhancing comfort promoting quality of life and fostering choice in end of life care for all patients and their families.

MINIMUM QUALIFICATIONS:
Education:

  • Nurse Practitioner:State of Ohio graduate of an accredited school of nursing.
  • Physician Assistant:Graduate of an ARC-PA accredited physician assistant program. Master degree preferred or eligibility for prescriptive authority under Ohio law.

Required Length and Type of Experience: 1 year experience in home health hospital or hospice/palliative care setting

Required Licensure Certification or Registry:

  • Nurse Practitioner:Current licensure by the Ohio State Board of Nursing
  • Physician Assistant:Current PAlicensure by the State Medical Board of Ohio

PRINCIPAL DUTIES AND RESPONSIBILITIES:
1. Performs and documents comprehensive patient assessments with particular focus on the Palliative patient.
2. Establishes and documents an appropriate diagnosis with an individualized plan of care incorporating goals of the patient and family while implementing interventions.
3. Offers educational development to staff patients and families within the specialty field of palliative and hospice care.
4. Manages a caseload of palliative care patients addressing the patients needs priorities of care and the need for collaborations as the patients conditions change and/or family goals are modified. Includes oversight of the patients medications and care plan delivery.
5. Functions as a consultant with nursing personnel regarding patient and symptom management for hospice and palliative patients confers with Medical Director or Managing Physicians as necessary.
6. Provides coverage in the absence of the Medical Director. Includes but is not limited to history and physicals face to face encounters on patients approaching the 3rd and subsequent benefit periods urgent visit needs and consults.
7. Manages oversight of the patients medications and treatments with prescriptive authority as outlined in State practice act.
8. Provides appropriate follow-up or referral to other health care providers.
9. Acts as a medical resource to the hospice interdisciplinary group.
