Job Summary: Provide occupational therapy services to patients in their place of residence who have impairments cognitive and/or physical resulting from injury disease disability or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient family physician and other allied health professionals to identify patient needs and coordinate services to meet those needs. Manager technical environmental and financial resources. Qualifications: 1- year certified home care OT Licensure Two years of experience in acute care rehab or homecare as an Occupational Therapist Registered with the Board of Registration in Massachusetts Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred Strong customer service skills and ability to work with diverse populations Excellent verbal written and electronic communications skills Ability to prioritize work and adapt to changing situations Travel required within PHHs geographic area
