The Program

Taproot Community Support Services will be offering a new Community Based Community Inclusion program called Bloom. This will be a non traditional style day program for Community Living British Columbia (CLBC) eligible adults who are over the age of 19. This program has capacity to serve 25-30 persons served. The Bloom Community Inclusion (CI) program will be located in the vicinity of Central City in Surrey BC. Bloom CI will be open for service 7 days a week from 9:00 am to 9:00 pm (except for statutory holidays). The Bloom program intends to promote the active and meaningful participation of individuals with diverse abilities in all aspects of community life. By doing so this fosters equity accessibility and respect. Blooms programming will help adults in service to achieve an enhanced quality of life receive personalized supports and gain access to new opportunities such as employment volunteer placements and a sense of belonging.

The Position

The Community Support Worker identifies and provides specialized supports and interventions for persons with developmental disabilities FAS/FAE and/or Autism Spectrum Disorder in an accredited agency. Supports and instruction focus on community inclusion skill development social network and employment skills. The positions will provide services to clients within the new Surrey based Community Based Community Inclusion program.

Scheduling:
Eight (8) permanent full-time positions that will each work 37.5 hours per week
Flexibility with the schedule will be required as shifts will be scheduled based on the needs of the individual between 0900 and 2100
The program will operate 7 days a week and shifts will be staggered. Services will be available from morning until the evening including weekends.

In Your Role You Will

Initiate contact with referred individuals. Assist the manager with intake procedures assess suitability for services assess support needs and formulate service plans with individuals.
Make referrals to other programs agencies and/or community resources as appropriate.
Assist individuals to plan goals and provides support in accessing community skill development employment skills guidance and emotional support. Monitor their well being and provide feedback and support to families of clients and other stakeholders in their lives.
Recognize and analyze potential emergency situations and develop strategies to deal with them. Utilize their knowledge of persons with developmental disabilities FAS/FAE and/or Autism Spectrum Disorder develop service plans and front line instruction around crisis management problem solving and community based instruction.
Develop the individuals network of community resources including natural supports to increase support opportunities for community inclusion beyond (some examples: connecting with ride share to attend community activities; finding a gym buddy; establishing peer support at college; links to community support groups such as social skills mental health and substance use supports).
Help individuals access resources in the community and provide referrals to services. Assist individuals to obtain volunteer placements or employment either directly or by coordinating with other agencies.
Support individuals who are active in addiction unhoused transient individuals with complex needs and have experience supporting individuals who may be complex and have many barriers
Provide services to Individuals who may require support with ADLs medication administration and other health related needs.
Provide information that helps individuals make better-informed choices.
Advocate for the individuals and assist them in self-advocacy.
Review individuals progress and make reports regarding their daily activities and progress.
Liaise with other agencies professionals Ministry staff and the community.
Develop Personalized Service Plans that are person centered and comprehensive in line with Dr. Shalocks Quality of Life Indicators.
Think outside the box for possible solutions and finding available resources
Perform miscellaneous job-related duties as assigned.

As An Ideal Candidate You Possess

A degree diploma or certificate in one of the humanities social work or related field. Other combinations of education and experience may be considered.
Minimum of one year experience working with persons with a developmental disability and multiple complex needs.
Valid CPR/First Aid certificate (or ability to obtain prior to employment)
Valid Class 5 BC Drivers License and current drivers abstract (hiring subject to abstract review)
Non-Violent Crisis Intervention Certificate (i.e. MANDT) certification (or ability to obtain during probationary period)
Successful candidate must sign an oath of confidentiality a conflict of interest statement abuse protocols and a code of ethics statement.
Must have consistent/reliable access to a computer/working printer/scanner/copier
Cellular phone with data plan (compensation provided for business use of personal device)
