Halton Healthcare supports system-wide improvements in patient access and navigation by leading and contributing to coordinated access pathways across the Ontario Health Central Region and its sub-regions including MSK Orthopedics Mental Health & Addictions (one-LINK) Diabetes and Diabetic Foot Care helping to ensure timely equitable and coordinated care for patients across the region.
We are seeking a Senior Project Manager who will play a central role in designing implementing and optimizing high-impact coordinated access initiatives. This role works closely with clinical teams system partners Ontario Health and community providers to support hospital modernization system transformation and improved access to care across the region.
Lead major regional and sub-regional coordinated access initiativesincluding MSK Orthopedics Mental Health & Addictions Diabetes and Diabetic Foot Careby managing the full project lifecycle and ensuring alignment with provincial guidelines and Ontario Health priorities.
Support governance structures and regional committees while facilitating collaboration with partner hospitals community agencies and primary care providers across the Central Region.
Translate coordinated access priorities into detailed project plans overseeing documentation tracking and performance reporting across all pathways.
Build and sustain strong relationships with clinical leaders community partners Ontario Health and patient/family advisors by leading meetings workshops and co-design sessions.
Lead change management planning and execution to support adoption of new coordinated access pathways and digital tools identifying risks and mitigation strategies to ensure successful implementation.
Prepare high-quality materials presentations and progress updates for executive-level committees and regional governance tables ensuring follow-through on decisions and action items.
Develop monitor and report on key performance indicatorsincluding referral volumes wait times equity measures and patient experienceto support continuous quality improvement and strategic decision-making.
Identify opportunities to improve referral management enhance patient navigation optimize workflows and support long-term sustainability and scalability across coordinated access pathways.
Qualifications :
Masters degree in Health Administration Public or Business Administration or related field; equivalent experience considered.
Undergraduate degree in health business or related discipline preferred.
PMP designation preferred.
Change Management Certification (e.g. PROSCI) an asset.
Minimum 5 years of experience in healthcare including work with cross-functional teams and complex multi-site projects.
Strong understanding of coordinated access models clinical workflow redesign and the Ontario Health/Regional structure.
Experience with strategic planning performance measurement and hospital/health system operations.
Demonstrated ability to navigate multi-stakeholder environments and build consensus.
Excellent communication skills with the ability to create high-quality reports presentations and executive-level materials.
Exceptional organizational analytical and problem-solving skills.
Proficiency in Microsoft Office suite particularly Excel PowerPoint and project management software.
Additional Information :
Hours: Monday to Friday 7.5 hour shifts 0800-1600 (hours are subject to change based on operational needs)
Salary: $58.769- $73.487
