In accordance with the vision mission and values of Resthaven the Care Partner will provide comprehensive coordination and individualized case management and planning in partnership with Support at Home individuals.
The Care Partner will oversee and assess the services provided monitoring outcomes against each individuals service plan with an aim to optimize a persons quality of life and achievement of goals.
Resthaven currently has an opportunity for a 12-month full-time contract at our Northern Community Services site at Elizabeth. In this role it is expected you will demonstrate personal responsibility effective communication and adaptability while consistently delivering high-quality service aligned with Resthavens values. This includes clear and respectful communication active listening and providing timely updates.
Selection Criteria
Essential
- Appropriately trained aged care workers with relevant experience.
- Program Coordination Team Leader case management experience
- An empathy and understanding of issues for older people their families and carers
- Knowledge and experience of quality systems for evidence-based care coupled with effective written and verbal communication skills
- A commitment to supporting older people to remain at home in the community
- Demonstrated ability to work in a holistic person centred model of care
- An ability to work with individuals to identify their needs develop personal goals and develop responsive packages of services
- Demonstrated computer literacy and familliarity with MS Office Suite and electronic client care systems
- A current SA drivers license
Desirable
. The following qualifications are highly recommended:
- Diploma of Nursing
- Certificate III in individual Support (Ageing) or Health Services Assistance
- Certificate IV in Aged Care Disability or Community Services
- Diploma of Community Services (Case Management)
- Diploma of Ageing Studies and services
