Job Description:

POSITION PURPOSE SUMMARY:

As a member of the health care team assists in the assessing planning and implementation and evaluation of resident care. Assumes significant responsibility in ensuring continuity of care through recording and reporting.

ACCOUNTABILITIES:

Clinical Responsibilities

Participates in shift report with oncoming and outgoing RPNs/PSWs

Participates in the planning and implementation of the residents individualized plans of care when care needs are well-defined with predictable outcomes

Consults with the RN when resident care needs become more complex with unpredictable outcomes

Provides and/or assists with the personal care of the residents as required.

Evaluates care levels and reassigns staff accordingly to assure provision of safe nursing practices.

Participates in care conferences

Reviews physicians orders; arranges for treatments medications and other therapeutic services as needed.

Delegates some aspects of care to PSWs but retains overall accountability for care

Documents Clinical Records in an accurate and concise matter in accordance with CNO Standards

Completes Incident Reports accurately and promptly

Assesses residents and reports promptly to the RN and POA/family member of any changes in a residents condition.

Administers medication controls narcotics in accordance with established procedures.

Counts narcotics with another Registered Nursing staff at the beginning/end of each shift

Seeks advice when required from the Registered Nurse prior to administering prn medications per policy.

Participates in Nursing Continuous Quality Improvement audits

Acts as a second relay in Resident call bell alarms. Monitors to ensure response time are appropriate.

Carries out nursing procedures in accordance with the College of Nurses of Ontario Standards and evidence-based practice

Demonstrates and shares nursing best practices in daily routines.

Provides assistance in taking residents to planned social and recreational activities.

Supervises and coordinates staff and workload of home area.

Under the direction of the RAI Coordinator participates in activities related to RAI MDS (ie. completing assessments coding RAPS care planning workload).

Completes clinical documentation to support RAI-MDS assessments coding and RAPS.

Quality Improvement/Risk Management:

Monitors efficient use of equipment and supplies on unit; reports equipment malfunction and/or supply needs to the Director of Environmental Services/Director of Resident Care.

Attends Staff Development and In-service Education programs to ensure personal and professional growth.

May be designated as an Area Warden in Fire/Safety emergencies.

Environment:

Maintaining a safe and secure environment for residents co-workers volunteers and visitors by following facility policies and procedures; strives to enrich the environment by minimizing the institutional setting and noise by facilitating a quiet calm and homelike environment.

WORKING CONDITIONS:

Long term care facility

May encounter disgruntled staff or resident/family representatives

Much of this position is on foot with minimal sitting

EDUCATION AND EXPERIENCE QUALIFICATIONS:

Education Qualifications and Certifications:

Current registration as a Registered Practical Nurse with the College of Nurses of Ontario.

Current CPR Certificate.

Experience in geriatric nursing considered an asset

Experience and Skilled Knowledge Requirements

Ability to safely and efficiently dispense resident medical and treatments

Demonstrated sound clinical knowledge/ judgement

Ability to organize workload using time management principles.

Excellent verbal and written communication skills in English.

Knowledge of computer operations and software applications.

A clear VSC with results acceptable to TSA completed in the last 3 months

A negative TB test either 2 step or chest x-ray completed in the last 3 months

Skills and Capabilities:

Must possess the personal qualities and attitudes that respect and maintain the spirit dignity and individuality of the residents.

Sound clinical judgment tact and personal ability to work cooperatively and effectively with residents family members physicians volunteers and co-workers as a member of an Interdisciplinary Care Team.

Self-motivated ability to work carry out responsibilities with minimum direction and to use good judgment in making appropriate decisions.

Must be physically and mentally able to carry out responsibilities and able to cope effectively with stress

Ability to develop and maintain rapport with elderly residents.

Ability to work cooperatively with all staff and maintain a working climate that is positive respectful and mutually supportive.

Works closely with other disciplines to ensure that residents have their physical psychological social and spiritual needs met.

Compensation:

The target hiring range for this position is $33.51 to $33.94 with a maximum of $34.36.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
