Position Overview

We are currently seeking a Living Healthy Services Manager to oversee implement and coordinate the delivery of our Integrated Chronic Disease Care South West program which aims to improve health outcomes through the provision of care coordination support for individuals with poorly managed chronic disease through a multi-disciplinary team.

Key Components of the Role

  1. Provide a collaborative and strategic approach to chronic conditions care that optimises the use of resources available to the South West region.
  2. Collaborate with stakeholders within the Community to ensure service delivery complement and enhance existing primary care services.
  3. Support General Practitioner (GP) led/informed care utilising team-based care.
  4. Coordinate care coordination to support overall client self-management.
  5. Ensure the delivery of consumer-centred care in a shared decision-making partnership between the client their family and carers and the referring GP.
  6. Program leadership of an existing nursing and allied health team based throughout the South West including Clinical Care Coordinators Diabetes Educators Exercise Physiologists and a Dietician.
  7. Work collaboratively with other Oseca services and Program Managers to provide coordinated services.
  8. Manage the preparation and submission of high-quality reports data collection and program evaluation.
  9. Review develop and implement guidelines policies and procedures for the program as required.
  10. Monitor on an ongoing basis the program ensuring it meets budgets and guidelines.

About You: You are a Registered Nurse with a background in clinical nursing within Aged Care Practice Nursing or similar environment with experience in supporting clients with Diabetes. You have strong leadership skills and a passion for clinical excellence. You have experience in managing teams and optimising clinical outcomes within budget constraints.

Essential Requirements:

To be considered for this role you must meet the following criteria:

  • Minimum degree qualifications as a Registered Nurse
  • Relevant postgraduate qualifications are strongly preferred
  • At least 5 years experience in a Clinical Nurse role
  • Demonstrated people leadership experience
  • Proven experience in chronic condition management.
  • An understanding of innovative care models integrated care or partnership-based service delivery.
  • Current Registration with AHPRA
  • The right to work in Australia (and be based in the South West currently)
  • Pre-employment Health Declaration
  • A valid WA drivers license
  • A valid and clear police check
  • A Working with Children Check WA (WWC)
