The Case Management Team Leader will work alongside the case management team supporting day to day operations while providing guidance structure and oversight. This role is not a hands off management position; instead the Team Leader will actively participate in department tasks assist team members improve processes and help ensure caseloads move smoothly and efficiently.
The ideal candidate is collaborative dependable and able to support staff while maintaining accountability and quality. Experience in HCBS ABA or related healthcare services is preferred.
Key Responsibilities:
Assist with daily case management workflow and serve as a point of support for team members
Help oversee department activities to ensure deadlines and quality standards are met
Provide guidance structure and mentorship to the case management team
Track caseload progress and follow up with staff as needed
Collaborate with supervisors to maintain efficient processes and compliance
Assist with communication between departments families and providers
Identify areas for improvement and support solutions to streamline operations
Qualifications:
Prior experience overseeing or supporting a team required
Experience in case management healthcare HCBS or ABA services preferred
Strong communication organizational and follow up skills
Ability to multitask in a busy environment
Team oriented reliable and proactive
