The Mississauga Halton Central Intake Program plays a pivotal role in advancing and streamlining access to a range of specialized healthcare services across the Mississauga & Halton and greater OH Central regions. As part of this program you will be involved in overseeing centralized intake screening and waitlist support across several key pathways ensuring that patients are efficiently connected to the appropriate care providers based on their specific needs. The pathways within the program that this role would be responsible for include:
a) One-LINK: This pathway provides centralized intake and screening for all LHIN-funded addictions and mental health service providers across Mississauga and Halton. One-LINK serves as a crucial entry point for individuals seeking support for mental health and addiction concerns ensuring they are connected to a wide range of community-based resources.
b) Diabetes & Diabetic Foot Care: This pathway focuses on providing comprehensive support for individuals living with diabetes. Responsibilities include intake clinical triage to assess patient needs and coordinating care plans and resource matching to connect patients with appropriate services and providers ensuring seamless access to diabetes management and specialized foot care to prevent and manage complications.
c) Self-Management: This pathway empowers individuals to take charge of their chronic conditions through education and support. It involves promoting self-management strategies for patients offering resources that help individuals manage their health through lifestyle changes and chronic disease management.
d) MSK Orthopedics Central Region Hub Lead: The OH Central Region Hub for MSK orthopedics provides centralized intake triage and coordinated waitlist management for orthopedic referrals across the Ontario Health Central Region. This includes ensuring consistent processes enhancing referral quality supporting equitable distribution of surgical consults across multiple hospitals and collaborating closely with orthopedic surgeons and hospital partners to improve patient access and reduce regional wait time variation.
As part of the team you will contribute to improving access to these essential services reducing wait times and ensuring a seamless connection between patients and the appropriate resources. Key Responsibilities:
- Leadership: Provide management direction and operational planning for the program(s) aligning service delivery with organizational goals and community/partner needs.
- Financial Oversight: Manage the program operating budget ensuring effective financial planning and resource allocation in accordance with organizational priorities.
- Quality Improvement: Ensure program alignment with the Quintuple Aim by streamlining service navigation to improve population health enhance patient experience reduce costs support care teams and advance health equity. Lead initiatives to enhance quality of care and patient outcomes integrating evidence-based practices and industry standards into service delivery models.
- Collaborative Engagement: Collaborate with key stakeholders and community partners to identify clinical priorities and improve service delivery through patient and family-centered approaches
- Partnership Development: Foster collaborative relationships with community partners to support integrated care
- Leadership in Transformation: Lead efforts to transform the coordinated access and central intake experience setting client-centric goals and fostering a culture of continuous improvement within the program. This includes optimizing referral processes enhancing service navigation and ensuring timely and equitable access to health services
Qualifications :

- Masters degree in healthcare administration Social Work Nursing or equivalent preferred
- Current registration with the College of Nurses of Ontario or other Health Regulatory Professional College mandatory
- Minimum of 5 years of clinical and administrative experience in a leadership position in healthcare and systems planning
- Demonstrated ability to think and plan strategically
- Excellent interpersonal skills and ability to work effectively with others and form productive partnerships

- Excellent communication (verbal and written) and presentation skills
- Proficiency in business management program planning human resources management analytical and budgeting skills
