On Lok PACE at IOA is seeking a Registered Nurse to join its team in San Franciscos Inner Richmond neighborhood. Reporting to the Director of Nurses the Clinic RN offers primary care nursing to a geriatric population in an outpatient long-term care setting.

On Lok pioneered the Program of All-Inclusive Care for the Elderly (PACE) a nationally recognized model that helps seniors live safely and independently in their own homes.

The Institute on Aging (IOA) operates an On Lok PACE Center in San Franciscos Richmond District where older adults who are ill or disabled can receive the care and support they need to remain at home for as long as possible.

At IOAs PACE Center participants enjoy a lively welcoming environment with access to medical care social activities exercise programs and nutritious meals all designed to support their health independence and quality of life.

Job Description

SPECIFIC RESPONSIBILITIES:

  • Implements directs supervises and evaluates nursing care for members of the On Lok PACE at the IOA.
  • Participates in care planning with the Interdisciplinary Team as assigned
  • Evaluates individual needs and participates in the development of a comprehensive nursing care plan.
  • Manages minor illnesses injuries and the stable chronic illnesses of participants according to standard policies and procedures.
  • Initiates preventative and rehabilitative nursing procedures as appropriate for the participants care and safety.
  • Evaluates acute illnesses in collaboration with the primary care provider.
  • Coordinates appropriate referrals to health care providers including follow-up.
  • Teaches supervises and counsels the participant and family members regarding nursing care needs and other related problems of the participant at home.
  • Meets consults and provides instruction for direct care workers as needed.
  • Reviews charts to coordinate participant care andensuresthat medical plans are executed.
  • Instructs assists and coordinates care with Clinic Aides Medical Assistants and LVNs.
  • Makes nursing home or home visits when appropriate and as directed by the physician.
  • Manages triage phone calls and coordinates participant discharge from acute and subacute facilities.
  • Effectively collaborates with interdisciplinary colleagues and diverse staff and participants.
  • Communicates effectively both verbally and in writing and provides and receives feedback in a professional manner.
  • Meets all the administrative requirements including statistical records medical records utilizing computerized system attending staff meetings etc.
  • Actively participates in and encourages actions that promote good public relations with participants their families and friends visitorsand the community.
  • All other reasonably related jobs as assigned.

REQUIRED QUALIFICATIONS:

  • Current California Registered Nurse license
  • Current BLS certification (CPR) or willingness to obtain one within 6-months of hire.
  • Ability to work within the interdisciplinary team with initiative imagination resourcefulness and flexibility.
  • Knowledge of the community served.
  • Interest and willingness to work in a multilingual multicultural environment.
  • One-year experience working with the elderly or frail required.
  • Ability to comprehend complex written materials and verbal instructions.

DESIRED QUALIFICATIONS:

  • Graduate of an accredited college or university with a baccalaureate degree in nursing preferred.
  • Possesses a passion for working with a geriatric population.
  • Computer literacy preferred.
  • Language capability Mandarin or Cantonese-speaking preferred.
  • Experience with diverse cultures and communities.
  • Knowledge of best practices for medical and behavioral interventions.
  • Demonstrated ability to be flexible in times of change.
  • Demonstrated excellence in working in a team environment.
  • EPIC electronic medical records experience preferred.

PHYSICAL REQUIREMENTS:

  • Must be able to lift transfer push/pull maneuver and reposition 35 lbs.
  • Ability to reach bend and walk
  • Five finger manipulation

COMPENSATION:

Range: $58 - $66/hourly
