- Pay Range: $16-17.50 hourly
Responsibilities
- Under direction ensure that the activities of this position and relevant programs are consistent with the mission vision and values of Catholic Charities Diocese of Cleveland.
- Screen telephone calls; take all telephone and written referrals. Respond to all phone inquiries as appropriate.
- Perform diversion screenings and develop a shelter plan for all individuals/families that have other housing options or who are in a non-emergency situation.
- Complete referrals to emergency shelter/crisis response providers via utilization of the Available Resources List for individuals/families that cannot be diverted from the shelter system.
- Assist staff as well as clients with completion of intake forms and documents.
- Work collaboratively with case managers within the shelter system.
- Collect and enter required Homeless Management Information System (HMIS) data.
- Manage referrals from Coordinated Entry program to appropriate emergency shelters within the Continuum of Care. Maintain referral tracking to emergency shelter programs.
- Prepare and complete all required case records administrative reports statistical reports and related paperwork and maintain confidential records and compliance with program evaluation procedures.
- Perform other duties as necessary to accomplish objectives.
Working Conditions and Physical Demands:
- Normal office environment.
- Travel throughout service area required.
- Frequent sitting talking listening/hearing for client interviews and answering phones.
- Reading writing and finger dexterity to manage correspondence faxing copying answering phones forms/documents computer keyboard and client files required on a frequent basis.
- Organizing and coordinating schedules.
- Analyzing and interpreting program data.
- Communicating with the public staff and community partners.
- Frequent use of computer and telephone/cell phone.
Requirements:
- Combination of experience and education normally represented by a bachelors degree in social services or related field with1-2 years case management experience with the homeless preferred.
- LSW preferred.
- Must be at least 21 years of age at the time of hire.
- Must have a working knowledge and utilization of community resources in Lorain County.
- Must have competent oral written and interpersonal communication skills.
- Must be well organized detail oriented dependable and can maintain confidentiality.
- Ability to work with diverse client population and community organizations as well as staff at all levels.
- Must be willing to work a flexible schedule that includes evenings holidays and weekends.
- Ability to show patience and courtesy when working with people is a necessity.
- Good working knowledge of computers with familiarity of Microsoft Office.
- Must have a valid Ohio Drivers License provide copy of drivers license and are subject to moving vehicle report (MVR) check through the Department of Motor Vehicles. Any MVR grade of unacceptable is not permitted to operate a company vehicle or personal vehicle on behalf of Catholic Charities.
- Must provide proof of auto insurance including effective dates of coverage and maintain minimum liability insurance coverage of $100000 per individual and $300000 per occurrence for driving personal vehicle.
- Final applicant is required to be fingerprinted to complete background check.
